# Chronic Radiation Effects Chronic radiation effects are the delayed health consequences that occur after prolonged exposure to ionizing radiation or after an acute exposure when sufficient time has elapsed for biological damage to become apparent. In the context of CBRNE (Chemical, Biological, Radiological, Nuclear and Explosive) emergencies, radiation exposure may occur through external irradiation, contamination with radioactive materials, or incorporation of radionuclides into the body. Unlike acute radiation sickness, which generally follows a relatively high dose received over a short period, chronic radiation effects are primarily associated with long-term, repeated, or relatively low-dose exposure. Their importance lies in the fact that clinical manifestations may develop months, years, or even decades after exposure. The biological effects of radiation arise mainly from damage to cellular DNA. Ionizing radiation can directly break DNA strands or indirectly cause injury through the formation of reactive oxygen species. Cells that divide rapidly or have limited capacity for repair are particularly vulnerable. The severity and probability of long-term effects depend on the radiation dose, dose rate, type and energy of radiation, duration of exposure, organs involved, age at exposure, and individual susceptibility. Repeated exposure can produce cumulative biological damage even when individual doses are too small to cause immediate symptoms. One of the most important chronic radiation effects is carcinogenesis. Ionizing radiation is a recognized carcinogen because radiation-induced DNA mutations and chromosomal abnormalities may initiate malignant transformation. Radiation-associated cancers can appear after a latency period ranging from several years to several decades. The risk increases generally with increasing cumulative dose, although there is no clearly established threshold for many radiation-induced cancers. Important malignancies associated with radiation exposure include leukemia, thyroid cancer, breast cancer, lung cancer, skin cancer, bone cancer, and certain gastrointestinal and other solid tumors. Leukemia often has a shorter latency period than many solid cancers. The thyroid gland is particularly susceptible to radioactive iodine exposure. Radioiodine may be released during nuclear accidents and can be inhaled or ingested. It is concentrated by the thyroid and may subsequently increase the risk of thyroid nodules and thyroid cancer, especially in children. Children are more vulnerable because of their smaller thyroid size, higher metabolic activity, and greater remaining lifetime during which radiation-induced cancer can develop. Appropriate public-health measures, including control of contaminated food and water and, when indicated, administration of stable potassium iodide under official guidance, can reduce thyroid uptake of radioactive iodine. Chronic radiation exposure can also affect the bone marrow and hematopoietic system. Persistent or repeated exposure may result in reduced blood-cell production, anemia, leukopenia, thrombocytopenia, or other abnormalities. Bone-marrow injury can increase susceptibility to infection and bleeding and may contribute to the development of leukemia. In individuals with significant radiation exposure, long-term complete blood counts and appropriate hematological evaluation may
